What the template covers
The Code itself is set by law, in the NDIS (Code of Conduct) Rules, and published by the NDIS Quality and Safeguards Commission. Providers don't rewrite it - they show their workers know it and follow it. This template restates the eight elements word for word, explains in practical terms what each one means for your workers, sets out how to raise a concern and what happens when the Code is breached, and ends with an acknowledgement each worker signs, and signs again when it changes.

- Purpose
- Who this Code applies to
- The eight elements of the NDIS Code of Conduct
- Reporting a concern
- Breaches of this Code
- Signing and re-acknowledging this Code
- Acknowledgement
Which NDIS rules it helps you meet
- The NDIS Code of Conduct - binds all NDIS providers and workers, registered and unregistered
- Core Module 1.5 - Violence, Abuse, Neglect, Exploitation and Discrimination (policies and practices that actively prevent harm)
- Core Module 2.7 - Human Resource Management (auditors commonly look for a signed acknowledgement on each worker file)
Who needs it
Every NDIS provider: registered or unregistered, a sole trader or a larger organisation. The Code binds unregistered providers and their workers too, and the NDIS Commission can act on a breach by any of them. For registered providers, a signed acknowledgement on each worker's file is the usual evidence that workers have been told about the Code and agreed to follow it.

Example Provider Pty Ltd - Code of Conduct
ABN 12 345 678 901 · Version 1.0 · Effective 1 June 2026
Purpose
Example Provider Pty Ltd is committed to delivering safe, respectful and high-quality supports to people with disability. This Code of Conduct sets out the standards of behaviour we expect of everyone who works for or on behalf of Example Provider Pty Ltd - including employees, contractors, students and volunteers ("workers").
It is built on the NDIS Code of Conduct, which applies to all NDIS providers and workers under the National Disability Insurance Scheme Act 2013 and the NDIS Code of Conduct Rules. Following this Code is a condition of working with Example Provider Pty Ltd.
Who this Code applies to
- All employees of Example Provider Pty Ltd, whether full-time, part-time or casual.
- All contractors, sub-contractors and sole traders engaged by Example Provider Pty Ltd.
- Students on placement and volunteers.
- Management and any person involved in the governance of Example Provider Pty Ltd.
The eight elements of the NDIS Code of Conduct
The NDIS Code of Conduct requires that workers and providers, in providing supports and services to people with disability, must:
- Act with respect for individual rights to freedom of expression, self-determination and decision-making in accordance with applicable laws and conventions.
- Respect the privacy of people with disability.
- Provide supports and services in a safe and competent manner, with care and skill.
- Act with integrity, honesty and transparency.
- Promptly take steps to raise and act on concerns about matters that may impact the quality and safety of supports and services provided to people with disability.
- Take all reasonable steps to prevent and respond to all forms of violence against, and exploitation, neglect and abuse of, people with disability.
- Take all reasonable steps to prevent and respond to sexual misconduct.
- Not charge or represent that a higher price applies for the supply of goods to a person with disability than to a person without disability, unless the higher price is reasonably justified.
The sections below set out, in practical terms, what each element means for everyone who works for or on behalf of Example Provider Pty Ltd.
1. Respect individual rights to freedom of expression, self-determination and decision-making
- Support each participant to make their own choices and decisions, and respect those decisions.
- Give participants the information and time they need to make informed choices.
- Respect a participant’s right to take everyday risks and to live the life they choose ("dignity of risk").
- Never make decisions for a participant that they can make for themselves.
2. Respect the privacy of people with disability

- Keep the eight elements word for word - they restate the law. Tailor the practical examples to the supports you actually deliver.
- Write out, where the template prompts you, how you handle a Code concern step by step: who receives it, how quickly it is acknowledged and escalated, and who decides what happens. Your contact details fill in automatically.
- Have every worker sign before they start - employees, contractors, volunteers and students - and again whenever the Code is updated, and otherwise at least once a year.
- File each signed acknowledgement on the worker's record.
A template is a starting point. An auditor looks for a document that matches how your business really runs and evidence that you follow it, so read it through and change anything that isn't true for you.
Common questions
Is this NDIS Code of Conduct template really free?
Yes. Build your pack on Bluetail - no account and no payment - and download the full Code of Conduct as an editable Word file with your business name and logo on it. The other documents in the pack are a one-off $65 to unlock.
How many elements does the NDIS Code of Conduct have?
Eight. Workers and providers must respect people's rights to freedom of expression, self-determination and decision-making; respect privacy; provide supports safely and competently; act with integrity, honesty and transparency; promptly raise and act on concerns about quality and safety; take all reasonable steps to prevent and respond to violence, exploitation, neglect and abuse; take all reasonable steps to prevent and respond to sexual misconduct; and not charge a higher price for goods to a person with disability unless it is reasonably justified.
Do unregistered NDIS providers need a code of conduct?
The NDIS Code of Conduct applies to unregistered providers and their workers as well as registered ones. A written worker code with signed acknowledgements is the simplest way to show your workers know it and have agreed to follow it.
Do workers have to sign the Code of Conduct?
The Code applies to workers whether or not they sign anything. A signed acknowledgement is how you show each worker has read it and agreed to follow it, and registered providers are commonly asked for one on each worker file at audit. The template includes the acknowledgement page.
