Purpose
Example Provider Pty Ltd is committed to delivering safe, respectful and high-quality supports to people with disability. This Code of Conduct sets out the standards of behaviour we expect of everyone who works for or on behalf of Example Provider Pty Ltd - including employees, contractors, students and volunteers ("workers").
It is built on the NDIS Code of Conduct, which applies to all NDIS providers and workers under the National Disability Insurance Scheme Act 2013 and the NDIS Code of Conduct Rules. Following this Code is a condition of working with Example Provider Pty Ltd.
Who this Code applies to
- All employees of Example Provider Pty Ltd, whether full-time, part-time or casual.
- All contractors, sub-contractors and sole traders engaged by Example Provider Pty Ltd.
- Students on placement and volunteers.
- Management and any person involved in the governance of Example Provider Pty Ltd.
The seven obligations
Everyone covered by this Code must, in providing supports and services to people with disability:
1. Respect individual rights to freedom of expression, self-determination and decision-making
- Support each participant to make their own choices and decisions, and respect those decisions.
- Give participants the information and time they need to make informed choices.
- Respect a participant’s right to take everyday risks and to live the life they choose ("dignity of risk").
- Never make decisions for a participant that they can make for themselves.

Example Provider Pty Ltd - Incident Management Policy and Procedure

Purpose
This policy explains how Example Provider Pty Ltd identifies, manages, records and reports incidents that happen in connection with the supports we provide. Managing incidents well keeps participants safe, helps us learn and improve, and meets our obligations under the NDIS (Incident Management and Reportable Incidents) Rules 2018.
What is an incident?
An incident is any act, omission, event or circumstance that occurs in connection with providing supports or services to a participant, and that has, or could have, caused harm. It includes:
- Acts by a participant that happen in connection with the provision of supports and that caused harm or risk of harm to another person.
- Injury to a participant or worker.
- Errors (for example a medication error) or near misses.
- Property damage, or a participant going missing.
Reportable incidents
Some incidents are "reportable incidents" that must be notified to the NDIS Quality and Safeguards Commission. Reportable incidents are those that happen, or are alleged to have happened, in connection with the provision of supports, being:
- the death of a participant;
- serious injury of a participant;
- abuse or neglect of a participant;
- unlawful sexual or physical contact with, or assault of, a participant;
- sexual misconduct committed against, or in the presence of, a participant, including grooming;
- the use of a restrictive practice in relation to a participant that is unauthorised or not in line with a behaviour support plan.
